Revoking consent

You may withdraw your permission for InstaChart to access, use or disclose your health information at any time. Here is exactly what that means and how to do it.

What revoking consent means

If you previously gave InstaChart permission to access, use, or disclose your health information through connected health information networks or related services, you may revoke that consent at any time.

When you revoke consent:

  • InstaChart will stop future access based on that consent
  • InstaChart will stop future disclosures based on that consent, except where permitted or required by law
  • previously collected information may still be retained, used, or disclosed where legally permitted and as described in our Privacy Policy and applicable records retention requirements

Withdrawing consent

By revoking access, you are withdrawing your consent for InstaChart to:

  • Access your health information from connected health plans
  • Store your personal health information on InstaChart servers
  • Share your information with the application that requested the connection

How to revoke your consent

Use whichever of these is easiest for you.

Option 1 — Email request

You can request account deletion or exercise any of your data rights by emailing privacy@insta-chart.ai.

Include the following in your request:

  • Your full name
  • Email address associated with your account
  • Date of birth
  • The specific action you’re requesting (revoke access, delete account, export data, etc.)

Contact us by phone at (281) 705-7869, Monday through Friday, 08:00 – 17:00 Mountain Time.

Have your full name, the email address on your account and your date of birth ready so we can verify the request.

What happens after you revoke consent

After your revocation request is received and processed:

  • InstaChart will mark your consent as revoked
  • future retrieval or exchange activity based on that consent will stop
  • your access to connected data retrieval features associated with that consent may be disabled
  • you may need to provide new consent before using certain services again

Data retention

After you revoke access or request deletion:

  • Active accounts — Data is deleted within 45 days of your request
  • Terminated accounts — Data is deleted within 45 days of termination
  • Dormant accounts — If your account has no activity for over a year, data is deleted within 45 days of the dormancy period
